Dear Field Instructor:

We are asking you to complete a self-assessment survey that seeks to identify and rank Field Instructor Competencies. We invite you to participate in this project by completing and submitting this brief questionnaire.

Your participation is critical to the success of the project. This survey should take no more than 10 minutes to complete, and will provide important information to assist the COSW in assessing training needs of our field instructors. This information will also be used in our self-study documents for reaffirmation of our accreditation with the Council on Social Work Education (CSWE).

Please be assured that your response is completely anonymous. Information from this project will be utilized in aggregate form only.
